Dear Student,

 

For joining the campus please fill [font]this form. Kindly note that you can join the campus in case you fulfil either of the following condition:[/font]

• You are completely vaccinated (any of the anti-Covid vaccines)
  
• At least one dose of vaccine + Negative report of rt-PCR (not more than 72 hours validity).
  

 

Please note:

• Avoid taking leave for short term visits unless there is an emergency.
  
• Please email me the scanned copy of the leave approval form duly forwarded by your supervisor and Head of the Department. 
  
• Only after the approval from the supervisor and Head of the Department I will approve your application to leave the campus.
  

 

The form will be open till 20/09/2021 (5:00 PM). You can join the campus from 13/09/2021.
